*BEVERLY HILLS CLASP**SERVICE COORDINATOR 1- OUTREACH**REPORTS TO:* Assistant Program Manager*HOURS:* Part-Time 9 am to 5 pm*CLASSIFICATION:* Not-Exempt*Location: *2025 W 3rd St, Los Angeles 90057*SUMMARY:* The Beverly Hills CLASP (Changing Lives and Sharing Places) program is funded primarily by the city of Beverly Hills and allows three two full time staff and two part-time staff to engage with 'Unsheltered persons' who are experiencing chronic homelessness and mental health conditions in the city of Beverly Hills. The Outreach Service Coordinator I will provide outreach services to engage homeless individuals with chronic mental illness and to provide necessary referrals to Los Angeles' most competitive and comprehensive Mental Health, Veterans and Housing programs. Step Up embraces Wellness, Rehabilitation and Recovery, Housing First and Motivational Interviewing and promotes the use of these principles and practices daily when interacting with clients.*DUTIES:* The following reflects essential functions for this job but does not restrict other tasks, which may be assignedDirect Service and Support Work as part of a multi-disciplinary team which will include the Lead Outreach Service Coordinator I, a Licensed Clinical Social Worker and the Human Services Administrator from the city of Beverly Hills, while serving adults on the streets of Beverly Hills and local surrounding community. Work shift which includes weeknights and Saturday and Sundays. Collaborate with Licensed staff to further evaluate for a qualifying mental illness diagnosis. This will include level of functioning, support network, adequacy of living arrangements, financial status, physical health, level of self-care, medication and treatment history and compliance. Evaluate for immediate needs such as crisis intervention, medical attention, showers, clothing, food, emergency shelter, etc. Make referrals as needed. Develop rapport and continue to make street outreach to same individuals while continuing to promote linkage to services. Assist with benefits establishment. Attend staff meetings, supervision and trainings as requested. Other duties as assigned.Documentation and Data Collection Become trained to administer and input CES packets. Maintain clear and timely submission of Community Outreach Services (COS) notes via Apricot HER system Complete timesheets and any recommended trainings in a timely manner*SKILLS: *Ability to engage homeless individuals on the streets in a meaningful and nonthreatening manner, evaluate and assess their needs and begin to develop trust enough that they accept supports and services offered. Knowledge of or experience with DSM V diagnoses and of level of functioning, DMH documentation, EBP' s such as Motivational interviewing, Trauma Informed Care, Housing First, Harm Reduction, etc. Should have the ability to work independently and collaboratively with team members, take initiative, and embrace a solution focused practice approach.*QUALIFICATIONS:* BA in psychology or social work, or 2 years of related work experience, with a minimum of 2 years' experience working with persons experiencing mental health disorders and homelessness, in a community setting. Case management experience is essential. Candidate must have own transportation and present evidence of current California driver's license and insurance. Position requires frequent driving.*SUPPLEMENTAL INFORMATION: *Work with people experiencing homelessness whether on the streets, in shelters or other places of habitation or services, may present inherent challenges and difficulties such as: exposure to bed bugs or other infestations, unpleasant smells or odors, individuals who may have poor hygiene or unclean homes due to homelessness, mental health symptoms, or poverty. Additionally, in working with our members, employees may encounter instances of profanity, sexually explicit or derogatory language, or verbal or physical expressions of anger and trauma. Very rarely do these situations ever escalate to physical contact, and negative or derogatory communication patterns can often be negotiated successfully. All direct care staff are provided adequate training to develop skills to deescalate crisis situations that may arise and Step Up has established procedural safeguards for all employees to mitigate (but not eliminate) these inherent risks of employment.*PHYSICAL REQUIREMENTS:* While performing the duties of this job, the employee is regularly required to sit; use hands and fingers; handle or feel; reach with hands and arms; talk; and hear. The employee is frequently required to walk, balance, stoop, kneel, and/or crouch. The employee must occasionally lift and/or move up to 15 pounds. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception, and ability to adjust focus.
