Palliative Care Physician - Covina, CA Job type: Full-time /
Permanent Shift: Days Job Description: We are seeking a
compassionate and board-certified Palliative Care Physician to join
our interdisciplinary team. This role offers the flexibility to set
your own pace with rounds, supported by a dedicated Nurse
Practitioner (NP) to assist in managing patient caseloads. You'll
provide specialized palliative care to patients in LTAC/STAC and
acute care settings, collaborating closely with an
interdisciplinary team to offer patient-centered care plans and
manage complex symptoms. Key Responsibilities: Lead patient care in
LTAC/STAC and acute care facilities, supporting clinical
decision-making and guiding patients and families through complex
medical situations. Manage serious illness symptoms, including
prescribing medications, and monitor treatment effectiveness in
collaboration with the NP. Oversee and direct the development of
proactive care plans, ensuring patient safety and optimal outcomes.
Conduct rounds with the support of a Nurse Practitioner (NP) who
will assist with patient visits, documentation, and follow-ups.
Qualifications: Doctor of Medicine (M.D.) from an accredited
institution Board Certified or Board Eligible in Palliative Care
Active DEA License and CA License Proven experience in LTAC/STAC
settings preferred Strong interpersonal and communication skills to
