Job Opportunity – Care Manager – Southern California Start Date: February 2, 2026 Location: Mission Viejo, California Compensation: Details provided to qualified applicants Schedule: Weekend rotation required; 40-hour orientation; patient ratio 1:25 Highlights: Acute care facility setting focused on care coordination and discharge planning Work with adult and geriatric patient populations with daily census up to 25 patients Epic charting system utilized facility-wide May float to nearby facility locations within the region Professional dress code: turquoise scrubs or business casual attire Up to 7 days of requested time off available during assignment Requirements: Active California state RN license (pending accepted if licensed by start date) Minimum 2 years acute medical Care Manager experience performing care coordination Previous travel experience required Epic charting system experience required Trauma Level I or II experience preferred Strong skills in patient assessment, care coordination, discharge planning, and family meeting facilitation Competency in arranging DME, SNF placement, and ambulance transport independently Knowledge of HMOs, IPAs, community resources, advance directives, and protective services processes Why It's a Great Opportunity: This Care Manager role offers the chance to make a meaningful impact on patient outcomes through comprehensive care coordination in an acute care setting.
