Registered Nurse (RN) | Case Manager Location: West Hollywood, CA Agency: Revolution Healthcare Services Pay: $2,529 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 1/26/2026 About the Position Job Opportunity – Case Manager/RN – West Hollywood, California Start Date: January 26, 2026 Location: West Hollywood, California Compensation: Details provided to qualified applicants Schedule: 40-hour work week; weekends required; 15-patient ratio during weekdays; no on-call required Highlights: Inpatient case management role with interdisciplinary support from Social Services and Transportation teams Epic charting system utilized facility-wide 16 hours of paid orientation to support onboarding and integration Floating within scope of practice may be required to meet facility needs Serves a diverse patient population including adolescents, adults, and geriatrics with a daily census of 255 Local candidates welcome; first-time travelers accepted Requirements: Active, permanent California RN license (temporary licenses not accepted) Minimum 2 years of inpatient case management experience (3 years preferred) Knowledge of HMO and Medicare guidelines for inpatient, home health, and SNF settings Epic experience strongly preferred BLS certification required Two supervisor references from within the last year Nursys report dated within the last two weeks showing all licensed states Willingness to float within scope to meet facility needs Why It's a Great Opportunity: This case management position offers the chance to coordinate care for a diverse patient population while working with experienced interdisciplinary teams.
