You will be responsible for post discharge phone calls to assist with recovery process. Document and follow patients as they transition from inpatient orthopedic fracture care to various care settings and ultimately to their home. Ensure appropriate follow-up appointments are in place. Ensure patient has transportation to all appointments needed and that there no other barriers for attending appointments. Document each call in CS Link and collect patient information in Red Cap for quality improvement outcome purposes.#jobs
